| Change of Address | Change of address forms must include signature of owner or agent. |
| Request for Change of Assessor's Records | This form can be used to change a name due to marriage or to remove a name due to death or court order. A copy of the marriage certificate, death certificate or court order must be included. |
| Request for Review | Property owners that feel the market value of their property (as shown on the tax statement) is incorrect may request a review of their property by one of our appraisers. This form may also be used to inform us if structures have been removed from a property. If the review is requested soon after a tax statement is received and for the current year's market value, please also see the information circular, Requests for Review of Property Values |

Request for Estimate of Additional Taxes Due to Farm/Forest Disqualification |
To be used by property owners that currently have their property under farm or forest deferral and would like an estimate of the additional tax that would be imposed if the property no longer qualified for the special assessment. Authorization to Disqualify Property From Farm/Forest Deferral and Impose Taxes |
This form is to be used ONLY by property owners that are requesting to have their property disqualified from farm or forest deferral and have the additional tax as a result of the disqualification added to the next tax statement. Application for Forest Land Deferral/Forest Land Management Plan |
Property with at least 2.00 acres of wooded land may be eligible for this special assessment program. Includes the Forest Land Management Plan for properties that do not currently meet the minimum stocking requirements. For more information, see our web page on Forest Deferral and FAQs. |
| Application for Farm Land Deferral | Land that is actively being farmed for profit but is not located in an exclusive farm-use zone may qualify for this special assessment program. For more information, see our web page on Farm Deferral and FAQs. |

Farm Gross Income Questionnaire |
Properties currently under special assessment as Farm Land not in an exclusive farm use zone must continue to meet minimum gross income requirements for 3 out of every 5 years to continue to qualify. For more information, see our web page on Farm Deferral and FAQs. |

Request to Combine or Segregate Tax Lots |
Tax Lots can be combined or separated for tax assessment purposes only using this form and meeting specific requirements. Using this form does not create legal parcels unless accompanied by a partition plat or lot line adjustment approved by Land Development Services. |

This form can be used to get an estimate of tax for a portion of a property. |
